Telehealth Informed Consent
Telehealth Informed Consent
Effective Date: July 16, 2026 — Last Updated: July 16, 2026
What is Telehealth?
Telehealth is the delivery of healthcare services using electronic communications — including secure video and asynchronous messaging — with a licensed provider who is not physically present with you.
Benefits
Telehealth can mean better access to care, less travel and wait time, more privacy, and treatment for conditions that can be safely managed remotely.
Risks and Limitations
- Information transmitted electronically may not be sufficient for all medical decisions
- Technology failures may delay or interrupt your consultation
- Without access to your complete medical history, there is a risk of adverse drug interactions or allergic reactions
- Telehealth is not appropriate for all conditions; your provider may determine that in-person care is necessary
Your Rights
- You will be notified of your treating provider's name, credentials, and state of licensure when your consultation is assigned and reviewed. You can view this information at any time in your Appointment Details on the patient dashboard.
- You have the right to request in-person care at any time. Choosing not to use telehealth will not affect your ability to seek care elsewhere.
- You have the right to withdraw this consent at any time without penalty.
- The same confidentiality protections that apply to in-person care apply to your telehealth visit.
Technology Failure
If your consultation is interrupted, contact support@stealth.health to reschedule. In a medical emergency, call 911.
Audio-Only Services
We currently offer video consultations only. Audio-only service, if added, will require separate consent.
Your Location
By proceeding, you confirm the state on your registration reflects your actual location when receiving care.
